Points to Note for Persons under Home Isolation (Including Those Pending Admission)
Tested positive using Rapid Antigen Test
For people who obtained positive results through Rapid Antigen Test (RAT) for the first time can report via the online platform "Declaration System for individuals tested positive for COVID-19 using Rapid Antigen Test" (https://www.chp.gov.hk/ratp). All reporting must be done on the day of testing or the day after. If individuals encounter difficulties in using the online platform, they can call the automated system at 1836 119 to register their identification document and telephone number. The Centre for Health Protection will contact the relevant person later to finish the report procedure.

Note: Relevant individuals, who completed initial reporting via the "Declaration System for individuals tested positive for COVID-19 using Rapid Antigen Test" as well as their household contacts that, after receiving an SMS with the reference number concerned, they should follow the link to log-in and upload their supporting documents (photo of the RAT result and/or identity document) as soon as possible, so as to facilitate a speedy epidemiological follow-up of the Centre for Health Protection for appropriate support for patients with higher risks. After reviewing the usage by members of the public after the launch of the system, with effect from 0:00 am, 14 March, logging-in to the linked website provided in the SMS will only be valid for 24 hours after the SMS was sent. Relevant individuals must upload the supporting documents within 24 hours upon receipt of the SMS, or else their log-in will become invalid and they would not be able to upload supporting documents with the link and download their isolation or quarantine order issued by the Centre for Health Protection in accordance with the law. In addition, they will not be able to report again via the system. With regard to SMS sent between 7 March and 0:00 am, 13 March, those links will also become invalid at 0:00am, 14 March.
For those who are tested positive using a COVID-19 nucleic acid test, please use the link provided in the SMS message informing the result for declaration.
Tested positive or preliminary positive by Nucleic Acid Test
People with positive or preliminary positive COVID-19 nucleic acid test results will receive an SMS notification about the result, together with a request to fill in an electronic form (https://www.chp.gov.hk/cdpi) to submit information, including those on family members living in the same household. This is to facilitate the work of the Centre for Health Protection such as epidemiological investigation and arrangement of hospital admission, isolation and quarantine. People who receive the relevant SMS should check the personal information and information on testing in the SMS carefully and fill in the electronic form as soon as possible to provide accurate and complete information.
- The hotline 1836 115 operates daily from 8 am to 10 pm seven days a week to answer enquiries on medical information and provide infection control advice for people under home isolation (including those pending admission to isolation facilities) upon confirmed infection

If you have the following warning symptoms, go to an Accident and Emergency (A&E) facility directly:
- Persistent fever (>38.5°C) for more than 2 days despite fever-reducing medicine*
- Hypothermia (≤35°C)
- Convulsion
- New onset of difficulty breathing or shortness of breath
- Loss of speech or mobility
- Inability to wake or stay awake
- Pain or pressure in chest or abdomen
- Dizziness or confusion or marked change in mental status
- Weakness or unsteadiness
- Decreased or no movement of fetus (for pregnant women at ≥ 24 weeks of gestation)
- Persistent inability to drink or eat, or diarrhea / vomiting for more than 2 days
